All adult soccer players are required to be "carded" with a soccer ID card produced by the Maryland SoccerPlex & Discovery Sports Center.  Only carded and rostered players are permitted to play in any match.  To get your card:

• Come to the Discovery Sports Center front office
• Bring a photo ID and $5.00
• Bring the completed waiver form (linked below)

Cost:

• $790 per team
• $80 per player (We will combine players who do not have teams to make a house team)*

• Adult Roster Form ( ALL teams must submit this form )
• Adult Waiver Form ( ALL players must submit this form.  All forms must be originals, no fax or scanned copies )
