Division: Men’s Open (18+ years old) 

 

Play: Three 30-minute games are guaranteed.  Play is 7v7.

 

Cost: $350 per team (cash, credit cards, or checks)

Times: 5:00pm - 12:00midnight

 

Registration Deadline: Friday, December 21 at 5:00pm

 

Prize: $1,000

** The champion will receive the $1,000 prize if 12 teams are entered in the tournament.  If less than 12 teams, the monetary prize will be adjusted accordingly **

 

 

 

Team Roster - Each team must provide a completed roster with all player names.

Individual Waiver Form - Each player listed on team roster must completely fill out a Waiver Form.
